PG'S 4th ANNUAL TOP WOMEN IN GROCERY AWARDS NOMINATION FORM 2010

Progressive Grocer is seeking your help in compiling our annual list of leading women in the supermarket industry, to be featured in our annual Top Women in Grocery issue. In a business where males have traditionally far outnumbered females, we recognize the women who are helping initiate change. These women represent all major sectors of the grocery industry - retailers, suppliers, and wholesalers - in three distinct categories:

1.) Senior Level Executives (Retailers, Suppliers, & Wholesalers)
2.)Rising Stars (Retailers, Suppliers, & Wholesalers; Job titles above Store Managers but below Vice President)
3.)Store Managers (Retailers only)

SELECTION PROCESS: We ask for nominations of the female movers and shakers who have demonstrated outstanding leadership and wielded substantial influence in their companies and in the industry at large during the previous year; and who are also deeply involved in the communities they serve and in philanthropic activities.

Please answer the following for Rising Stars and Senior Level Executive Candidates:

What is the economic sphere she controls, i.e. how much money\budget does she control, or how much money does she help generate for the company, etc?*

What were her achievements at the company financial or otherwise between January 1, 2009 and December 31, 2009?*

Emphasize specifics about what the nominee has achieved in those 15 months to merit being named a Top Women in Grocery. Please focus on what she achieved and worked on in that timeframe even if it relates to an ongoing initiative that was started before then.*

Please answer the following for Store Manager Candidates:

What store does she manage?*

How many employees does she oversee?*

Please describe the candidate's history in the company and what led to her role as store manager.*

What were her main achievements as store manager between January 1, 2009 and December 31, 2009?*

Please share any examples that demonstrate her innovation and commitment.*
